Citizenship Measurement Tool
Funding: Yale University School of Medicine. The development of this tool is part of an ongoing effort to develop a model of Citizenship-Oriented Care, integrating the basic principles of Citizenship into case management activity and treatment planning. The tool serves as a guide for case managers to help clients explore the “5 Rs” (as defined by Institute Research Consultant Michael Rowe): the rights, responsibilities, roles, resources, and relationships available to them. This study determined that using this tool led case managers to a greater understanding of the challenges clients face, as well as the ways in which direct care staff can promote wellness by helping their clients discover and develop new identities, new roles, new relationships, and opportunities to connect with their communities.
