The New Haven Family Partnership

The New Haven Family Partnership (NHFP), formerly of ALSO-Cornerstone, is a collaboration between The Connection, Inc., New Haven Home Recovery, Inc. and The Coordinating Council for Children in Crisis, Inc. It is designed to provide permanent supportive services to families where the head of household suffers from mental illness, chronic substance abuse, dual diagnosis, or HIV/AIDS and who are homeless or at risk for homelessness. Participants must also either have their children in their custody or be in the process of reunification within three months of the admission date.
 
The client must meet at least one of the following criteria:
  • Be a recent recipient of a housing subsidy, such as, Shelter Plus Care, Section 8, RAP, etc.
  • Meet income requirements for occupancy of one of the subsidized apartments accessible through this program (this applies only to those who are not recipients of a housing subsidy)
  • Meet the eligibility requirements and go through a screening process to receive a housing voucher through Shelter Plus Care
  • Clients will not be required to pay more than 30% of their income towards rent for any of the three housing situations.

Each family is provided with a Case Manager and a Family Specialist who help determine client needs and assist clients in accomplishing their goals.

The Case Manager is responsible for coordinating needed services including but not limited to: establishing safe and affordable housing; obtaining supports to improve the likelihood of recovery and maintained housing; accessing services for any co-existing problems such as: domestic violence, financial, educational, or medical problems, linking clients to services for on-going crisis and relapse prevention, accessing vocational services, obtaining entitlements and budgeting.

The Family Specialist will coordinate and provide direct home-based services that meet the educational, health, and therapeutic needs of the children in the household. The degree of severity, duration, and the level of services from the Family Specialist will be determined on a case-by-case basis.
